POLIO: OPV or IPV; Must receive three or more doses with last dose being a booster and having been received on or after 4th birthday.

DPT: Must receive 4 or more doses of DPT with last dose being a booster and received on or after the 4th birthday.

Td: If the child is 6yrs old or older when they enter school and they have not had DPT as a pre-school student, they must have 2 doses of  Td no less then 4 wks apart with a booster a year later.

MMR: Must receive 2 doses  after 12 months of age.

Hib: Required for children 2 yrs of age or older entering school-operated program below kindergarten level. Any child who has had one Hib Vaccine on or after 15 months of age is considered in compliance with the Hib Vaccine requirement.

Hepatitis B: This is a series that is a requirement for all children entering 5th grade and children 2 yrs of age or older entering a school-operated program below the Kindergarten level.

Varicella: one dose of chickenpox vaccine (varicella) on or after 1st birthday. This is required for kindergarten, 1st, and 2nd grade students. A physician can confirm past disease history in lieu of vaccination, by having examined the infected child, documenting the parent/guardian's description of the child's history, or reviewing laboratory evidence.

Tdap:  one dose of Tdap for students in Grades 6-12.  For students entering Grade 6 and Grade 9 for the 2012-2013 school
year, parents must show proof that the Tdap vaccination was given. To see what proof you must provide, please click here
