TO PARENTS OF KINDERGARTEN
STUDENTS:
The State of Illinois
requires every child entering kindergarten to meet specific medical
requirements. Those include a physical exam, vision exam, dental
exam, lead assessment by his/her doctor, and up-to-date immunizations.
Every child entering kindergarten is required to present a copy
of their certified birth certificate when registering for school.
The physical exam form is two-sided. The top half of the back side of
the form must be filled out and signed by the parent/guardian.
The following are the
required immunizations for school attendance:
Measles,
mumps, and rubella vaccine (MMR): Two doses of measles vaccine are
required. The first dose needs to be given
on or after the first birthday and the second dose no less than
four weeks (28 days) after the first. One dose of rubella and mumps
vaccine is required on or after the first birthday.
DPT/DtaP:
Four or more doses with the last dose qualifying as a booster and
received on or after the 4th birthday.
Polio:
Three or more doses with the last dose qualifying as a booster and
received on or after the 4th birthday.
Hib: At
least one dose of Hib vaccine at 15 months of age or older and by 59
months of age.
Varicella:
One dose of chickenpox vaccine (varicella) on or after their first
birthday. A physician can confirm past disease
history-in lieu of vaccination-by having examined the infected
child’s history, or reviewing laboratory evidence.
Your child can be immunized
by his/her doctor or at the Public Health Department (348-0530). Make
your appointments early so you don’t get caught in the last minute
rush! The physical may be dated 1 year prior to the first day of
school, the dental exam may be dated on or after November 15, 2008, and
the vision may be dated on or after October 15, 2008. Completed
vision, dental and physical exams with lead assessment and immunizations
should be brought to central registration in early August. These health
requirements MUST be turned in before the student can begin
kindergarten.
