Dear Parent/Guardian:

If your child will need to take any medication at school, you will need to provide a completed medication form for each medicine. Please click on the links on the left side of this page, for the correct form for your child.  You, your doctor, and I will all need to sign the medication forms.

All completed forms can be brought to me at the time of central registration which is held in August. Or you may also bring them to your child’s school or to the high school health office.
