Students must have the following forms on file with the athletic department prior to participating in open gyms, tryouts, or practice with an extracurricular/athletic team:

IHSA Residence Eligibility Form

(Freshmen and transfer students only)

IHSA Preparticipation Examination

(Physicals are valid for one calendar year)

Charleston CUSD #1 Insurance Waiver 

In addition, students may COMPETE in interscholastic events only after a completed

CUSD #1 Activity Participation Fee Form (Print Only) 

is submitted  to the athletic office.
